Cardiac Output and Totl4l Peripheral Resistance in Post-hemorrhagic Hypotension and Shock1

چکیده
The anesthetized dog when submitted to a prolonged period of drastic hypotension, achieved by controlled bleeding, enters into a state of irreversible shock. The standard procedure developed in this laboratory for producing this state (1) ent!ails lowering the arterial blood pressure by rapid bleeding to 50 mm. of Hg and sustaining it at this level for 90 minutes; it is then reduced to 30 mm. of Hg, at which level it is maintained for an additional 45 minutes. At the conclusion of this 135 minute interval of severe hypotension, all withdrawn blood is rapidly reinfused. Despite the latter procedure, verv fendogs have actually survived. Studies have also been conducted by members of this laboratory (2) to ascertain the nature of the permanent derangement produced by severe hemorrhagic hypotension. Ho th peripheral and cardiac events were followed during the development and terminal stages of the shock state. they used to measure cardiac output, though more satisfactory from many standpoints for th is purpose than gasometric methods, unfortunately necessitated the The cardiometric method which

Studies in Clinical Shock and Hypotension. Ii. Hemodynamic Effects of Norepinephrine and Angiotensin.
Although vasoconstrictor drugs have been widely used for many years in the treatment of hypotension and shock, their clinical value is still the subject of much controversy (1). Norepinephrine has been recommended for support of the blood pressure in shock occurring in a wide variety of clinical settings (2-5), but Nickerson (6) claims that its vasoconstrictor effect is deleterious and that it ...
متن کاملInfluence of population and exercise protocol characteristics on hemodynamic determinants of post-aerobic exercise hypotension
Due to differences in study populations and protocols, the hemodynamic determinants of post-aerobic exercise hypotension (PAEH) are controversial. This review analyzed the factors that might influence PAEH hemodynamic determinants, through a search on PubMed using the following key words: "postexercise" or "post-exercise" combined with "hypotension", "blood pressure", "cardiac output", and "per...
متن کاملCardiovascular effects of dobutamine stress testing in women with suspected coronary artery disease.
OBJECTIVE To investigate the central and peripheral haemodynamic response to dobutamine stress testing in women with suspected ischaemic heart disease and to seek an explanation for the hypotension phenomenon. DESIGN 18 women aged 54-73 years were investigated with Doppler echocardiography and venous occlusion plethysmography during intravenous infusion of dobutamine 5-10 d after an episode o...
متن کاملHemodynamic response to abdominal aortotomy in the anesthetized swine.
This study was conducted to determine the hemodynamic response to uncontrolled hemorrhage following aortotomy in anesthetized swine. Eight Yorkshire swine underwent splenectomy and stainless steel wire placement in the anterior infrarenal aorta and were instrumented with Swan-Ganz and carotid artery catheters. Following an equilibration period, the wire was pulled. This produced a 5 mm aortotom...
متن کاملHypotension during cemented arthroplasty. Relationship to cardiac output and fat embolism.
An episode of hypotension is common during cemented joint replacement, and has been associated with circulatory collapse and sudden death. We studied the mechanism of hypotension in two groups of six dogs after simulated bilateral cemented arthroplasty. In one group, with no lavage, the insertion of cement and prosthesis was followed by severe hypotension, elevated pulmonary artery pressure, de...
